At Sand Lake Surgery Center, we are committed to making surgical care accessible to all members of our community. We understand that medical expenses can be overwhelming, and we will provide support to help patients who are experiencing financial hardship.
If the patient informs Sand Lake Surgery Center that they cannot pay their patient responsibility at the time of service, there are 3rd party programs such as Care Credit available that the patient can apply for and make monthly payments.
All credit programs, website links, and telephone numbers will be provided to the patient along with a brief explanation of the program and application process.
Eligibility Criteria:
- Financial assistance is available to uninsured, underinsured, and low-income patients who meet specific guidelines.
- Eligibility is determined through a confidential review of income, assets, and household size through a 3rd party source (Care Credit).
How to Apply:
- Patients may request a Care Credit application during registration or download it from our website.
